What is our Reablement Service?

Skillcare operates the Reablement programmes throughout North and North West London hospital. If you would benefit from additional care once being discharged from hospital, then we would be assigned to provide it.

Reablement care is designed to help you relearn how to do things for yourself. We will work with you to improve your level of independence to ultimately keep you in your own home for as long as possible.

Our service is intended to help you regain the capacity to do things like the washing up, cooking meals, or getting around your house again, and it will include practical ways to help you restore mobility, confidence and life skills.

ABOUT OUR PRIMARY FOCUS

Our primary focus will always be how much can you do for yourself and how much extra help do you require. This will be unique to you. Our staff will implement strategies discussed within the team to help you achieve this. On each visit, the carers will choose which battles to fight to maximise the benefit to your independence, while being equally aware not to push too hard. Our carers will take a holistic view of how to help the whole family. Sometimes the most significant pressure is felt by your primary family carer. The carer’s role can often be to help your family and friends to get themselves back fully functioning.

Reablement care is designed to last for up to six weeks at which point it is expected that we will have a good idea of what your long-term care needs are likely to be.

Our staff are all trained in practical enablement techniques that are designed to make the process of improvement easier and more likely to succeed. We will assess your needs and discuss what you wish to achieve. We will then together devise a plan taking into account strength, stamina and dexterity requirements.

All our experienced care staff all have the following outstanding qualities:

  • 1. Patience
    It is the ability to seem unrushed, untroubled by time and not to be too ambitious that you feel uncomfortable with the speed of progress. They need to be able to help you pick the right projects no matter how small and allow you the time and space to restore your health.
  • 2. Empathy
    In reablement, the ability of the carer to see it from your point of view is vital. If we do not understand both your mental and physical condition, we don’t know what the right goals and strategies that will work for you.
  • 3. Confidence Building
    A period of time in hospital, or an illness, injury, or disability can shatter your confidence. Our carers are trained to help you build up your confidence and motivation to want to get stronger.
  • 4. Intuition and Common Sense.
    A good reabling carer will know the right way to support you. They can sense how far to progress. They will also have good judgement in choosing the right tactics for facilitating and encouraging improvement.

The scope and the pace of the service are driven purely by you, the client. No task is too small. We work collectively with other specialists within the reablement team to achieve your outcomes.
